“HEART TOMORROW” Patient electing to preserve their fertility by freezing their eggs
- Any cash-paying patient who is electing to freeze her eggs for fertility preservation is eligible, as long as she is prescribed 25 vials of Menopur or more
- The 25 vials must be written on her first Rx. Any additional vials in any future prescriptions within that cycle are also eligible
- Application and terms can be found at: https://www.ferringfertility.com/paying-for-treatment/save-on-your-medication/heart-tomorrow
- The patient fills out the application, indicating the number of vials (25 or more) and the pharmacy (any Menopur pharmacy)
- The application is signed by patient and doctor
- The patient sends application to Ferring at hearttomorrow@envisionrx.com
- Ferring then notifies the indicated pharmacy that this patient’s price for Menopur is $68/vial and that’s what the pharmacy bills the patient
“HEARTBEAT” Onco-fertility patients considering fertility preservation services prior to cancer treatment
Qualifying patients must be:
- Female
- US citizen or permanent resident
- Diagnosed with cancer
- Without chemotherapy within the previous 6 months
- Fill out and send attached application to AllianceRx, as indicated (note, this is the only pharmacy able to adjudicate this specific program)
“HEART FOR HEROES” IVF for Veterans and their spouses
- Ferring is committed to helping eligible veterans and their spouses navigate the road to parenthood by offering IVF medication at no cost
- he Heart for Heroes program is for medically-separated from active duty Department of Defense category 2 or 3 veterans with a service-related injury resulting in infertility and requiring assisted reproduction, or patients and/or spouses with no insurance coverage for IVF medications
- Qualifying patients must be residents of the 50 United States or the District of Columbia
